Physical Forms

Sports Physicals generally require a physical form to be filled out by the primary care physician. This process takes anywhere from three to five days to prepare and return it to you. Please bring your forms in early so that you can have them returned  before your deadline.

 Walk-in Clinic

If your child has an acute illness, we invite you to come to our walk-in sick clinic Monday-Friday at 7:45 AM.   Patients will be scheduled on a first-come, first-served basis.  We will be happy to schedule an appointment later in the day if we are unable to see your child during walk in clinic.
