The Modified Checklist for Autism in Toddlers (M-CHAT) is a screening tool we use in our office at the 18 and 24 month well visits. You can complete the questionnaire in advance of your child's checkup by downloading here and bringing the printed form to our office on the day of your child's visit.
We use the Ages and Stages Questionnaire at each well visit from age 2 months through 5 years. Please click on the appropriate link below to access the questionnaire appropriate for your child. These questionnaires may be printed and completed and brought to your child's checkup.
